Forms for Initial Session

Prior to your first session at Rasnic Counseling, we need you to complete a few forms for our records.  If you are unable to download and complete these prior to your session, please plan to arrive 20 minutes early to fill out the necessary paperwork.

Category One:  Individual and Marital Counseling
